The Mil Mi-8AMTSh is the dedicated armoured armed assault version of the Mil Mi-8AMT. The Mi-8AMTSh is fitted with aramid fiber plates, which are placed around the cockpit and engines. The Mi-8AMTSh's first flight was in 1996, but it's first publicly demonstrated flight was three years later, in 1999. The first helicopters were delivered to the Russian Air Force in 2009 and has been in service ever since.

The Mil Mi-8AMTSh is a better armoured, better armed version of its sister aircraft in its series. The main role of the aircraft is to carry cargo and troops, while supporting them with it's firepower. It is also used for evacuations and Combat S&R (Search & Rescue). It can be used to soften enemy defences before friendly troops land. It has a crew of 3, one of which is a gunner responsible for the controlling and using the aircraft's weapons system. For its troop transport, it can carry 26 troops or 12 stretchers with a single medical attendant in the cargo area, which can be easily transformed for medical evacuations. It can also carry 4000 kg of payload inside its cargo compartment, it can also carry loads of up to 4000 kg externally. It is fitted with an updated and more powerful engine than its sisters. It also has a structurally improved rotor blades.

Most of its armament is derived from that of the Mi-24, its two stub-wings has a total of 12 hardpoints for its various types of weapons. It can carry lots of different types of guided missiles, unguided rocket pods and machine guns. There are two 7.62 mm machine guns hidden behind the front doors that can be operated by passengers. The Mi-8AMTSh can carry the Shturm-V and Ataka-V anti-tank missiles, it can also carry the Igla-V air-to-air missile.
